- For every insurance-based appointment, there are some mandatory information needs to be entered in the claim form.
- So, to add this information, click on Patient list > Select a Patient > Patient Demographics menu.
- In the Additional Information tab, we have listed some of the information to be captured in the claim form.

- The details that were added in the Additional Information page are:
- Referring Provider
- Referring NPI
- Referral Number
- Referral date
- Referral Expires
- Onset Date of Current Illness
- Illness Date
- Unable to Work Date
- Hospitalized with Illness Date
- Condition related to current or previous employment
- Condition related to an accident or an auto accident
- The details added in the Additional information page will be automatically populated in the claim form when the user if needed.
- As we are getting this information as default information, every insurance-based appointment claim will have the same information.
- The provider can edit this information in the editable claim form manually and click “Save” to save it based on “Globally” or “Claim Only”.
